In addition to providing real-time chart access when and where it’s needed, EpicCare Link also allows users to:
- Send and receive referrals
- Communicate directly with Memorial Hermann’s providers and Referral Center
- Place certain orders
- Send and receive messages via Epic’s InBasket messaging system
- Receive real-time event notifications for patient events (such as admission or discharge)
- Receive results via email or text message

EpicCare Link
EpicCare Link is a web-based tool that enables community practices and partners outside of Memorial Hermann to access shared patient information for better communication, care coordination and visibility to care transitions.
EpicCare Link complements but does not replace existing electronic health records (EHRs).
External groups that collaborate with Memorial Hermann, including physician practices not moving to Epic, community surgeon staff, billers, coders, research study monitors and others. Access is granted only to those external users with whom a Memorial Hermann patient has a relationship.
EpicCare Link is free to join.
EpicCare Link provides secure, read-only, real-time access to your Memorial Hermann patients’ full charts in Memorial Hermann’s Epic data repository – including lab results, diagnostic reports, hospitalization records, procedural information, discharge instructions, medications, allergies and medical history. If you are an OPID or SMR provider, please contact your Physician Liaison to learn more about the capabilities EpicCare Link will provide.
EpicCare Link offers a wide variety of features designed to improve care transitions; streamline referrals, labs and imaging processes; and reduce administrative costs and lead times.
To enroll, navigate to epiccarelink.memorialhermann.org and select New User. Once you accept the Terms and Conditions, and Confidentiality Agreement, complete and submit the online enrollment form. During this time, you will be required to enter a unique code to verify your eligibility to participate in EpicCare Link. Your unique code is MHEPICCARELINK. Following submission, you will receive login instructions along with your username and password via email within three business days.
No classroom training is required for use of EpicCare Link. Once you have gained access to EpicCare Link, a number of tutorials are available to help new users navigate the platform.
Your organization will have access and logins to EpicCare Link one to two weeks after signing up. Full functionality and use of EpicCare Link will be available starting May 22, 2024.
By using the Happy Together Link Activity, users may switch Link accounts seamlessly without having to log in or out.
No action is required from your patients.
