Life Flight air ambulance offers life-saving transport services within a 150-mile radius - 24 hours a day, 7 days a week, weather permitting. Private Learjet services are available for transportation needs beyond the greater Houston area, allowing for a higher level of care while en route to Memorial Hermann-Texas Medical Center.

How to Transfer a Patient to the Red Duke Trauma Institute

The Memorial Hermann System Transfer Center is designed to provide referring physicians and facilities quick and efficient transfer services for both adult and pediatric patients. By calling (713) 704-2500, your facility can quickly access our vast array of specialty services and renowned programs.

Follow these easy steps to transfer a patient:

  1. Call (713) 704-2500 or 1 (800) 392-5433.
  2. Provide your contact information and requested patient information:
    • Name, DOB, weight
    • Referring physician name and immediate contact number
    • Diagnosis and reason for transfer (which service you cannot provide)
    • Transfer type – Non-Emergent or Emergent
    • Patient status – Inpatient, OBS or ED
  3. Our Transfer Center Specialists will coordinate a conference call between the referring and accepting physicians. Inpatient transfer requests are screened clinically and financially prior to approval.
  4. Notify our specialist if you would like to arrange air transport via Life Flight® or ground transport with our Pediatric or Neonatal Transport Teams.
  5. Fax demographic sheet to (713) 704-3537.
  6. Complete required forms and prepare copies of the patient’s chart and all pertinent diagnostic studies related to the reason for transfer.
