Welcome to Terry Health Center!
Established in 2012, Memorial Hermann Terry School-based Health Center proudly serves the Lamar Consolidated Independent School District.
General Information
Our office is closed for lunch Monday through Friday from 12:00 PM to 1:00 PM.

Surface lot parking available.
Appointment Preparation
- Complete and sign the formsat the link below for your child to receive services at the health center.
- Make sure your child is prepared for the type of appointment they are coming to:
- Medical – Please send a list of any medications your child is taking, including over-the-counter and prescription drugs.
- Please send all previous immunization records and medical records to assist us in updating your child’s health.
- Nutritional – Make sure your child fasts for the first appointment. No eating after midnight the night before.
- Dental – Please bring any appliances and/or provide as much dental history as you can on your child.
- Write down any other concerns or questions you have so we can address them at your child’s appointment.

Preparación de Citas

- Médico – Envíe una lista de los medicamentos que toma su hijo, incluidos las medicamentos recetados y de venta libre.
- Envíe todos las registros de vacunas y registros médicos anteriores para ayudamos a actualizar la salud de su hijo.
- Nutricional – Asegúrese de que su hijo esté en ayunas para la primera cita. No comer después de la medianoche de la noche anterior.
- Dental – Por favor traiga todos las aparatos y/o proporcione tantos antecedentes dentales como pueda sabre su hijo.
