General Information

Memorial Hermann Sports Medicine & Rehabilitation Greenway Plaza/Upper Kirby offers a wide array of services from general therapy to specialty programs, with highly skilled and certified therapists. As part of patient-centered care, therapists perform comprehensive evaluations to determine appropriate treatment to individually address your specific goals.

Please note that all patients under the age of 16 are required to be accompanied by an adult, adult family member or legal guardian during their treatment time.

Patients 16 years of age or older are required to be accompanied by an adult during their initial appointment for paper work and consent purposes. After the initial visit, adult supervision may no longer be required.

If an adult is not present with the patient as listed by our age requirements, the patient may be asked to be rescheduled.

Physical & Occupational Therapy Services

Direct access provides you with an immediate physical therapy option. You can schedule an initial evaluation at one of our Sports Medicine and Rehabilitation, Memorial Hermann | Rockets Sports Medicine Institute, TIRR Memorial Hermann Outpatient Rehabilitation or Memorial Hermann Rehabilitation-Katy Outpatient Rehabilitation facilities to determine if physical therapy is the best treatment option for you.

Parking Information

Surface parking is located in front of our suite. Patients can also park in the bank or Costco area. There are limited Memorial Hermann Reserved spaces in front of the clinic. There is additional garage parking with Memorial Hermann spaces on the west end of the 3rd floor or the garage. When parking in the garage you are not required to park in the Memorial Hermann spots. There is only a breezeway connection to the front of the center from the 3rd floor.

If parking in the garage, you will walk through the breezeway next to Buffalo Wild Wings. There is an elevator on the Weslayan side to the north of the stairs. You may also take the stairs to the first floor. If you park on the lower level of the garage, you will need to walk around the building to get to the entrance of our suite.
